Medical practice certificate is a mandatory document according to regulations to prove that it meets the operating conditions of the clinic. However, during record keeping, this certificate may be lost or damaged. In this case, organizations and individuals in need need to carry out procedures to re-issue medical practice certificate according to regulations. Current regulations on medical practice certificate re-issuance for clinic facilities due to loss or damage are as follows:
1. Procedures
– Step 1: The medical examination and treatment facility sends an application for an operating license (GPHD) to the Department of Health.
– Step 2: If all documents are valid, the Department of Health receives and sends the application receipt to the applicant.
– Step 3: The Department of Health reviews the application and appraises it at the facility.
+ If the dossier is complete and valid, the Department of Health will establish an appraisal team and conduct an appraisal at the medical examination and treatment facility to issue an operating license. Implementation time is within 60 days for hospitals, 45 days for other forms of medical examination and treatment, from the date of receipt of complete documents in Step 2.
+ In case the application is not valid, within 10 working days from the date recorded on the application receipt form, the Department of Health must issue a written notice and request to complete the application. The time to process this case will be calculated from the date of receipt of complete additional documents.
+ In case of not issuing a driving license, the Department of Health must send a written response and state the reason.
– Step 4: The Department of Health returns the results to the applicant.
2. Dossier to request medical practice certificate re-issuance for clinic
– Application for an operating license;
– The original license is damaged (if any).
3. Implementation method
Individuals and organizations request to choose one of the following forms of procedure implementation:
– Submitting application directly.
– Submitting online via public service portal.
– Submitting by mail.

4. Result
License to operate medical examination and treatment.
5. Processing authority
The Director of the Department of Health re-issues medical practice certificate for clinic in the area.
