ISSUANCE OF MEDICAL PRACTICE CERTIFICATE OF GENERAL CLINIC

In order for general clinic to operate legally, it must meet the full conditions prescribed by law in terms of facilities, technical regulations, human capacity… When there is a need, the facility must carry out procedures with the competent authority to obtain medical practice certificate, satisfy legal requirements and at the same time commit to responsibility for the life and health of service users. Regulations on issuance of medical practice certificate of general clinic are as follows:

1. Competent authority to issue medical practice certificate of general clinic

Department of Health.

2. Implementation procedures

Step 1: The facility submits an application for medical practice certificate of general clinic by one of the following methods:

– Online;

– Direct;

– Through postal service.

Step 2: The Department of Health receives and sends the Receipt Form according to regulations to the applicant.

Step 3: Within 45 working days from the date of receiving complete documents, the Department of Health will review the documents and appraise them at the facility to issue medical practice certificate of general clinic:

– In case the application is not valid, within ten (10) working days from the date recorded on the application receipt, the Department of Health must notify the facility in writing to complete the application. Time to process procedures will be calculated from the date of receipt of complete additional documents.

– After sixty (60) days from the date of the written request, if the facility does not supplement/amend or the dossier does not meet the requirements, the entire procedure must be repeated from the beginning.

– In case the dossier is complete and valid, the Department of Health will establish an appraisal team and conduct the appraisal at the facility.

– In case of not issuing medical practice certificate of general clinic, the Department of Health must respond in writing and state the reason.

Step 4: Return result to the facility.

A facility applying for medical practice certificate of general clinic needs to submit 01 set of documents, including:

6	List of technical expertise of the proposed medical examination and treatment facility;	x
7	Decide to establish; business registration certificate; Investment registration certificate for foreign-invested establishments; or equivalent documents	x
8	Other relevant documents.
